29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide abatement documentation and certification for abatement of citation 1 item 1 in which employees were not supplied fall protection. Condition observed on or about 6/11/14.  b) The employer failed to provide abatement documentation and certification for abatement of citation 1 item 2 in which the employees were not provided fall protection training. Condition noted on or about 6/11/14.  c) The employer failed to provide abatement documentation and certification for abatement of citation 1 item 3 in which employees engaged in improper use of ladders and were exposed to fall hazards. Condition observed on or about 6/11/14.  d) The employer failed to provide abatement documentation and certification for abatement of citation 1 item 5 in which employees were provided with portable ladders with structural defects. Condition observed on or about 6/11/14.  e) The employer failed to provide abatement documentations and certification for abatement of citation 1 item 6 in which the employer did not provide ladder safety training for each employee. This employee had not received any fall protection training from the company.  Condition noted on or about 06/11/14. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a- 28 North Beach Drive, Ortley Beach NJ: Employees were observed using an aluminum extension ladder to access and egress the roof of a residential home.  The ladder's side rails did not extend three feet above the roof's landing.  As such, these employees were exposed to fall hazards up to 17 feet 5 inches to the ground surface. The ladder's end was visibly sinking into the sand base it was set in causing the ladder to be unstable.  As such, these employees were exposed to fall hazards up to 17 feet 5 inches to the ground surface.  Condition observed on or about 06/11/14. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a- 28 North Beach Drive, Ortley Beach NJ: Employees were observed using an aluminum extension ladder to access and egress the roof of a residential home.  The ladder visibly contained damaged rungs, was missing one of the fly section locking mechanisms and pull rope, and was missing its feet.  As such, these employees were exposed to fall hazards up to 17 feet 5 inches to the ground surface.  Condition noted on or about 06/11/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. As such, these employees were exposed to fall hazards up to 17 feet 5 inches to the ground surface.  These employee had not received any ladder safety training from the company.
