29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  a) Jobsite: Ground level - On or about 6/12/14, the flexible cord used to power the Sawzall reciprocating saw was not visually inspected for external defects before each day's use, or removed from service until repaired or replaced.  The outer sheath of the flexible cord was torn in four sections uncovering the energized conductors and exposing employees to potential shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.   a) Jobsite: Roof, 4th floor - On or about 6/12/14, no means of fall protection was used to protect employees from falling off the unprotected open sides and edges of the residential construction building.  The employees were performing framing operations approximately 30 feet above ground level.  WVM Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 966236, citation number 1, item number 3, and was affirmed as a final order on 6/5/14, with respect to a workplace located at 16 Cedar Grove Rd, Margate City, NJ 08402.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them.   a) Jobsite: Roof, 4th floor - On or about 6/12/14, the employer did not provide a training program for each employee in the recognition and avoidance of fall hazards during residential construction operations.  WVM Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 966236, citation number 1, item number 4, and was affirmed as a final order on 6/5/14, with respect to a workplace located at 16 Cedar Grove Rd, Margate City, NJ 08402.
