29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: Drydock Number 1: On or about June 10, 2014 and at times prior thereto, an employee performing maintenance on the exterior (waterside) wingwall of the floating drydock was elevated above the water in the basket of a "JLG." The employee was not wearing a body belt with a lanyard attached to the boom or basket. This condition exposed the employee to fall hazards up to 30 feet. General Industry standard applicable to Shipyard employment.

This condition exposed employees to fire hazards resulting in smoke inhalation injuries.
