29 CFR 1915.56(e): Arc welding and cutting operations were not shielded by noncombustible or flameproof screens to protect employees in the vicinity from the direct rays of the arc: Drydock #1, Tustumena: During arc welding repair work of the access ramp, shielding was not provided for employees accessing the ramp. This condition exposed employees to eye hazards.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): (a) Tustumena, Port Side: On or about June 13, 2017 and at times prior thereto, top rope rails used for guard rails measured 43 inches and deflected from 41 to 67 inches. Mid-rope rails measured 23 inches and deflected 16 to 37 inches in height. This condition exposed employees to a fall hazard of 26 feet. (b) Tustumena, Starboard Side: On or about June 13, 2017 and at times prior thereto, top rope rails used for guard rails measured 41 inches and deflected from 39 to 58 inches. Mid-rope rail did not exist. This condition exposed employees to a fall hazard of 26 feet.

29 CFR 1915.75(b): Railings on gangways or ramps were less than approximately 42 inches in height Drydock #1, Access Ramp: On or about June 15, 2017 and at times prior thereto, the guardrail of the access ramp measured 22 inches. This condition exposed employee to a fall to the water below.

29 CFR 1915.132(d): Worn and or frayed electric cables were not taken out of service: Drydock #1, Tustumena, Starboard Side: On or about June 13, 2017 and at times prior thereto, flexible cable used for grinding operations was frayed exposing conductors. This condition exposed employees to electrical shock hazards from contact with the exposed 120 volt circuit.
