OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  (a)    AMHS Tazlina, Pilot House Stairway:  On or about June 14, 2017 and at times prior thereto, the 35 inch wided fixed stairway of the vessel with 8 risers was not equipped with a hand rail.  This condition exposed employees to a fall hazard.  (b)    AMHS Tazlina, MOD 18 Stairway:  On or about June 14, 2017 and at times prior thereto, the 33 inch fixed stairway of the vessel with 15 risers was not equipped with a hand rail.  This condition exposed employees to a fall hazard.   Among other methods, a means of correcting this hazard would be to provide temporary railings.
