The employer did not ensure that all employees on the roof were tied off, on or about June 20 2014.  (b)  1618 West Thistle Drive, Wyomissing Pennsylvania - Four employees worked on a residential roof on two anchor points more than 6 feet above the ground. the employer did not ensure that each anchor point would support 5000 pounds per employee attached, on or about June 20, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  ATF CONTRACTOR LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 558258, CITATION NUMBER 1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 10, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT WYNGATE AT MEDFORD, LOT 44, 42 MOORLINCH BOULEVARD, MEDFORD NJ, 08055.
