29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 100 Independence Court, Blandon, PA; Employees were tearing off roofing shingles at heights between 11 feet 5 inches and 18 feet 5 inches without the use of fall protection, on or about 12/16/2015.    ATF Contractor, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 982326, citation number 1, item number 1 and was affirmed as a final order on 02/13/2015, with respect to a workplace located at 1618 West Thistle Drive, Wyomissing, PA 19610.   ATF Contractor, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 558258, citation number 1, item number 2 and was affirmed as a final order on 9/10/2012, with respect to a workplace located at Wyngate at Medford, lot 44, 42 Moorlinch Boulevard, Medford NJ, 08055.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) 100 Independence Court, Blandon, PA;Employees were using a Werner 20 foot Extension Ladder that was not placed 3 feet above the landing to access a roof work area, on or about 12/16/2015.  ATF Contractor, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 981386, citation number 1, item number 2 and was affirmed as a final order on 11/10/2014, with respect to a workplace located at 1618 West Thistle Drive, Wyomissing, PA 19610.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
