29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:Note: Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift: a) Jobsite, 2113 Race Street - Employees were installing windows from an unprotected platform placed in the forks of a JLG (model G6-42P, serial number 0160025666) telescopic forklift up to 22 feet above the ground level without fall protection, observed on or about 6/23/14.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite, 2113 Race Street - Employees were installing windows from an unprotected platform placed in the forks of a JLG (model G6-42P, serial number 0160025666) extensible boom up to 22 feet above the ground level without being trained in scaffold safety. The exposed employees did not know the trigger height for fall protection from scaffolds on or about 6/23/14.
