29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a. Stonewater Creek Subdivision, Lot #11A, 2062 Stonewater Creek, Hoschton, GA (Jobsite) - Employee were exposed to fall hazard. Employees were installing shake siding to the exterior of the newly constructed residential home while working on a ladder jack scaffold approximately 18 feet from ground level, without utilizing a means of fall protection, on or about July 16, 2014.

Stonewater Creek Subdivision, Lot #11A, 2062 Stonewater Creek, Hoschton, GA - Employees were exposed to fall hazard. Employees used the top section of the 30ft. aluminum portable extension access ladder as a crawling chicken board ladder which was used as steps to gain access to an upper level roof area, on or about 07/16/14.
