29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:    (a) New residential construction located on Lot #110, North side: On or about 03/02/2018, and at times prior, the employer exposed employees to collapse/fall hazards of 16-feet to the lower level, in that the bridging of the two aluminum scaffold platforms did not occur over a support were the connection was made as employees were engaged in siding installation.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    (a) New residential construction located on Lot #110, North side: On or about 03/02/2018, and at times prior, the employer did not perform frequent and regular inspection of the jobsite, equipment, and employees as employees were exposed to fall hazards of up to 16-feet, in that the scaffold platform was not supported were platforms verlapped and fall protection was not utilized while installing siding from a pump jack scaffold platform.       GCA Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1208020, citation number 1, item number 1 and was affirmed as a final order on 03/06/2017, with respect to a workplace located at Lot #72 6040 Harbor Mist Drive, Flowery Branch, GA.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) New residential construction located on Lot #110, North side: On or about 03/02/2018, and at times prior, the employer exposed employees to falls of 16-feet to the lower level, in that the employee installing siding on a pump jack scaffold platform was not connected to the life-line provided and was exposed to falls.     GCA Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(I), which was contained in OSHA inspection number 1207658, citation number 1, item number 3 and was affirmed as a final order on 03/06/2017, with respect to a workplace located at Lot #65 6012 Harbor Mist Drive, Flowery Branch, GA.
