29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a.  Harbour Lights Subdivision, 6040 Harbour Mist Drive, Lot #72, Flowery Branch, GA (Jobsite) - Employees (Roofing Subcontractors) were exposed to fall and struck-by hazards while working on the newly constructed residential home. The employer did not perform frequent and regular inspections of the project and/or the employees making sure the employees/roofers were utilizing their fall protection equipment and other protective equipment,  on or about February 3, 2017.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  Harbour Lights Subdivision, 6040 Harbour Mist Drive, Lot #72, Flowery Branch, GA (Jobsite)- Employees were exposed to struck-by hazard.  Employees were in the process of nailing down roofing material such as, felt paper and shingles and other roofing materials, using a pneumatic air gun, without the use of any eye protection, on or about 02/03/17.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501:    a.  Harbour Lights Subdivision, 6040 Harbour Mist Drive, Lot #72, Flowery Branch, GA (Jobsite) - Employees were exposed to fall hazard.  The employees were not utilizing a means of fall protection while working and walking approximately 28 feet above ground level on the roof, with a 8:12 pitch slope,   Installing roofing materials such as shingles and felt paper, along the upper level, on or about 02/03/17.  GCA Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 998001, citation number 1, item number 1 and was affirmed as a final order on November 24, 2014, with respect to a workplace located at 5340 Wild Oak Way, Buford, Georgia 30518.
