29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Front porch roof, new residential construction located on Lot #156 Still Pine Bend, Smyrna, GA 30080: Employee was exposed to overhead struck-by hazards while working directly below a ladder jack scaffold system without a protective helmet. The employee was exposed to stacked Nichi Board Shake Siding and a pneumatic tool while adjusting the ladder supports directly below the materials resting on the scaffold platform.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Front porch roof, new residential construction located on Lot #156 Still Pine Bend, Smyrna, GA 30080: Employees were exposed to falls while working on a 4/12 pitched roof at heights of approximately 9-feet, 9-inches installing siding with no fall protection system in place. GCA Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 998001, citation number 1, item number 2(a) and was affirmed as a final order on 11/25/2014, with respect to a workplace located at 5340 Wild Oak Way, Buford, GA 30518.
