Condition observed on or about 7/23/14. b) the employer failed to provide documentation and certification for abatement of citation 1 item 2 in which employees were not protected by personal fall arrest systems or guardrail systems. Condition observed on or about 7/23/14. c) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which the employer did not provide a training program for each employee potentially exposed to fall hazards. Condition observed on or about 7/23/14.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a- 5 Montclaire Road, Medford NJ: Employees were exposed to electrical shock when utilizing an orange extension cord, powering a Makita 7 1/4" electric circular saw, that contained several areas where the orange outer sheathing (and several wires within it) were damaged. Condition observed on or about 07/23/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The exposed and the other worksite company employees had not received any fall protection training from the company. Condition observed on or about 07/23/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
