29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which employees were exposed to electrical shock while utilizing orange extension cord that are worn or frayed. Condition observed on or about 7/23/14.  b) the employer failed to provide documentation and certification for abatement of citation 1 item 2 in which employees were not protected by personal fall arrest systems or guardrail systems. Condition observed on or about 7/23/14.  c) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which the employer did not provide a training program for each employee potentially exposed to fall hazards. Condition observed on or about 7/23/14.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a- 5 Montclaire Road, Medford NJ: Employees were exposed to electrical shock when utilizing an orange extension cord, powering a Makita 7 1/4" electric circular saw, that contained several areas where the orange outer sheathing (and several wires within it) were damaged.   Condition observed on or about 07/23/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 5 Montclaire Road, Medford NJ: An employee, not supplied with fall protection, was exposed to fall hazards of approximately 10 feet 2 inches to the ground surface below when performing floor sheathing installation, exterior wall framing and other related activities from the upper floor a residential structure.  Condition observed on or about 07/23/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- 5 Montclaire Road, Medford NJ: An employee, not supplied with fall protection, was exposed to fall hazards of approximately 10 feet 2 inches to the ground surface below when performing floor sheathing installation, exterior wall framing and other related activities from the upper floor a residential structure.  The exposed and the other worksite company employees had not received any fall protection training from the company.  Condition observed on or about 07/23/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
