29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                             A) On pump jack scaffold, right side of house: An employee was exposed to eye/face injuries while using a pneumatic nail gun and was not protected with any type of eye/face protection, on or about June 16th, 2014.   B) On pump jack scaffold, back of house: An employee was exposed to eye/face injuries while using a pneumatic nail gun and was not protected with any type of eye/face protection, on or about June 16th, 2014.

29 CFR 1926.451(g)(1):   Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A) Scaffold/ Right side of house - An employee was exposed to fall hazards of approximately 21 feet while working on a pump jack scaffold that was not protected by a complete guardrail system (including end rails) or other means of fall protection in place, on or about June 16th, 2014.   B) Back of house: An employee was exposed to fall hazards of approximately 21 feet while working on a pump jack scaffold that was not protected by a complete guardrail system (including end rails) or other means of fall protection in place, on or about June 16th, 2014.

The extension ladder that was used did not extend at least 3 feet above the landing, on or about June 16th, 2014.
