29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about October 2, 2014, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing employees to the hazards associated with eye injuries. 29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  On or about October 2, 2014, at the above addressed jobsite, employees were exposed to electrical shock/electrocution hazards when using extension cords which were plugged in to a portable generator. A ground fault circuit interrupter was not used and the employer did not institute an assured equipment grounding conductor program.  There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Instance 1.     On or about October 2, 2014, at the above addressed jobsite, employees constructing an exterior wall on a residential property were not protected from falling, exposing employees to fall hazards of approximately fourteen (14) feet.  Instance 2.    On or about October 2, 2014, at the above addressed jobsite, guardrails were missing from the second floor windows while employees were discarding material and debris through the windows, exposing employees to fall hazards of approximately fourteen (14) feet.  There is no abatement certification or documentation required for this item.
