On or about January 7, 2021 at jobsite located in 995 S. Mitchell Ave., Elmhurst, IL 60126, Employees were exposed to eye injuries while installing plywood with nail gun and utilizing a saw. Eye protection was not being utilized while employees engaged in this activities.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure. On or about January 7, 2021 at jobsite located in 995 S. Mitchell Ave., Elmhurst, IL 60126, employees were exposed to fall hazards of approximately 12 feet while engaged in residential construction activities. Fall protection was not being utilized while employees performed framing operations. A&F CONSTRUCTION CARPENTERS, CORP., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1148931, citation 1, item number 1 and was affirmed as a final order on August 8, 2016 with respect to a workplace located at 4641 Forest Ave., Downers Grove, IL 60515. A&F CONSTRUCTION CARPENTERS, CORP., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1143655, citation 2, item number 1 and was affirmed as a final order on August 8, 2016 with respect to a workplace located at 4446 Wilson Ave., Downers Grove, IL 60515.
