29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 29, 2015, at the above addressed jobsite, employees engaged in residential carpentry operations were exposed to fall hazards greater than six feet.  Instance 1.    Employees constructing the ridge and rafters on a residential property were not protected from falling, exposing employees to a fall hazard of twelve (12) feet.   Instance 2.    Employees performing residential carpentry operations were not protected from falling to the basement through the floor hole on the first floor, exposing employees to a fall hazard of eight (8) feet nine (9) inches.   A&F CONSTRUCTION CARPENTERS, CORP., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 997903, citation number 1, item number 3 and was affirmed as a final order on January 27, 2015, with respect to a workplace located at 316 Park Ave., Saint Charles, IL 60174.  There is no abatement certification or documentation required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about January 29, 2015, at the above addressed jobsite, the portable ladder used to access the working level did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls of nine (9) feet seven (7) inches.  A&F CONSTRUCTION CARPENTERS, CORP., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 997903, citation number 1, item number 4 and was affirmed as a final order on January 27, 2015, with respect to a workplace located at 316 Park Ave., Saint Charles, IL 60174.  There is no abatement certification or documentation required for this item.
