29 CFR 1910.147(e)(3): Specific procedures and training  were not developed, documented and incorporated into the employer's energy control program for removal of a lockout or tagout device under the direction of the employer when the authorized employee who applied the lockout is not available to remove it.    Army & Air Force Exchange Service, facilities: Specific procedures and training were not developed, documented and incorporated into the employer's energy control program for removal of a lockout or tag out device under the direction of the employer.  Employees used lock out during servicing of equipment and there were extra keys for the locks that could be used to remove the locks by someone other than the employee who applied it.
