29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about November 10, 2014, at the Stamping Press PR-2, a maintenance employee was exposed to a pinch point hazard when the electrical energy source to the Coil Feeder Unit was not isolated to make an adjustment on the tension of the pulley belt. Takumi Stamping, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 980824, citation number 1, item number 4a and was affirmed as a final order on December 1, 2014, with respect to a workplace located at 8585 Seward Road, Fairfield, OH 45011.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about November 10, 2014, at the Stamping Press PR-2, a maintenance employee was exposed to a pinch point hazard and by not affixing a lockout device to the electrical energy source to the Coil Feeder Unit while making an adjustment on the tension of the pulley belt.
