29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.      a) On August 24, 2017 an employee was injured when struck by a 2167 pound (983 kilograms) coil of steel and did not receive adequate first aid treatment, in that in lieu of 911 being called the employer called UBER to transport the employee to the hospital.  It took UBER 31 minutes to transport the employee from the plant to the hospital.  The plant nurse (plant designated trained first aid responder) was not onsite at the time of the injury to render the appropriate first aid and treatment.

29 CFR 1910.179(b)(6)(ii): Where passageways or walkways were provided, obstructions were so placed that the safety of personnel was jeopardized by movements of crane(s)      (a) On August 16, 2017 thru August 23, 2017 the employer did not ensure that the wood that was used during the maintenance of press PR 15 was not obstructing the path of the crane operator when loading coil rolls of steel, in that it was placed in front of the gate in the work area causing a trip hazard.

29 CFR 1910.217(f)(3): Work area. The employer shall provide clearance between machines so that movement of one operator will not interfere with the work of another. Ample room for cleaning machines, handling material, work pieces, and scrap shall also be provided. All surrounding floors shall be kept in good condition and free from obstructions, grease, oil, and water.      a)  On August 16, 2017 thru August 23, 2017 the employer did not ensure that the floor in front of the mandrel loading area was free from obstructions in that a stack of wood was in the area causing a trip hazard to the crane operator while loading the coil steel onto the mandrel.

29 CFR 1910.217(e)(1)(i)(A): Conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment;    a) The employer did not ensure that adequate daily press inspections of the PR 15 work area was being conducted, in that the wood stacked near the mandrel cage had been the area for at least 5 days (from August 16, 2017 thru August 23, 2017) creating a trip and fall hazard for employees loading the coil steel onto the mandrel using the overhead crane.
