Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a.) On or about December 2, 2022, the employer did not ensure employees standing in front of the load end the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor (Serial # 545228-2) located in front of aisle L1 secured long hair back to prevent entanglement.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. a.) On or about December 2, 2022, the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor (Serial # 545228-2) located in front of aisle L1, did not have a guard enclosing the tail pulley for the belt on the load side of the conveyor located 33 inches from the ground exposing employees with long hair to an in-running nip point. b.) On or about January 19, 2023, the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor (Serial # 545228-2) located in front of aisle L1, did not have an adequate guard covering the tail pulley for the belt on the load side of the conveyor to enclose an in-running nip point in that there was a 1 inch opening located 34 inches from the ground. c.) On or about January 19, 2023, the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor (Serial # 545228-2) located in front of aisle L1, did not have a guard covering the head pulley for the belt on the discharge side of the conveyor to enclose an in-running nip point in that there was a 1.5 inch opening located 33 inches from the ground. d.) On or about January 19, 2023, the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor located in front of aisle K1, did not have an adequate guard covering the tail pulley for the belt on the load side of the conveyor to enclose an in-running nip point in that there was a 1 inch opening located 34 inches from the ground. e.) On or about January 19, 2023, the Roach Model 450 BOS Box Style Slider Bed Belt Conveyor located in front of aisle K1, did not have a guard covering the head pulley for the belt on the discharge side of the conveyor to enclose an in-running nip point in that there was a 1.5 inch opening located 33 inches from the ground.

A black flexible cord was plugged into a 120 volt outlet located at ceiling height and was connected to an outlet strip at ground level to supply power to a computer, two display monitors, two PCS scanners, and an internet switch all located at the inspection conveyors in front of aisles L1 and K1.
