29 CFR 1910.147(e)(2)(i): The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On September 9, 2016 while setting up Spot Welder MW #21, the work area was not checked and employee positioned and/or removed from the area to prevent employees exposure to pinch points, in that while the machine was being set up, the machine operators are next to and/or in the area of the closing tip.

29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; a) On September 9, 2016 while Spot Welder MW #21 was being set up, the machine operator was not removed from the area, in that the operator was standing in the cell while the set up employee lowered the ram exposing them to pinch point hazards.
