29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.   (a) Building 65, Exit Door Number 11: On or about January 12, 2015 and times prior thereto, an exit route of hallways and passageways was not maintained free and unobstructed. The route was blocked with pallets loaded with machine components, portable grinder, portable belt sander, spanco hoist stand on wheels, a metal table, and thirteen wooden boxes.  Employees were exposed to acute smoke inhalation and inhalation burns.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Building 65, Machine Shop: On or about January 12, 2015 and times prior thereto, a Delta Disk Sander Serial Number 89053122, P/A Number 116441, did not have a guard to protect unused portion of the rotating disk. Employees were exposed to catching their fingers and hand at the unguarded disk.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  (a) Building 65, Machine Shop: On or about January 12, 2015 and times prior thereto, unused portion of the rotating parts such as unused drill bit, drill chuck, and spindle were not guarded on the Delta Dill, 55530, Serial Number 1057223.  Approximately 8 inches of the unused portion of the rotating parts were not guarded. Employees were exposed to fractures, ligament damage, and strain fingers related injuries.  (b) Building 65, Machine Shop: On or about January 12, 2015 and times prior thereto, unused portion of the vertical saw blade was not guarded on DoALL Band Saw, Model Number 3612-3, and Serial Number 153-701109. Approximately 3 inch of the saw blade exposed on the left and right side of the guide roller was not guarded. Employees were exposed to amputation related injuries.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not  constructed so hat the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel that never exceeds one-fourth inch:                                                                                                                                                                 (a) Building 65, Machine Shop: On or about January 12, 2015 and times prior thereto, Delta Abrasive Wheel Pedestal Grinder, Model 23-902 and Serial Number W1288, was not provided with a tongue guard that was ¼ of an inch from the abrasive wheel on the right and left sides. The left side of the wheel opening was measured at 2 inches and the right side was measured at 1.25 inches opening. Employees were exposed to foreign particles embedded to body and eyes.
