One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Building 472, Machine Shop: On or about January 14, 2015, a Bridgeport Milling Machine, Model: Ram, Serial Number J116421, Voltage: 230, and Amp 4.4 did not have a guard to protect the rotating parts. Employees were exposed to catching their fingers and hand at the unguarded disk. (b) Building 472, Machine Shop: On or about January 14, 2015, a Cincinnati Drill Press, Model 24 inch pedestal, Serial Number 2L3674, Voltage: 208-220, and Amp 15.2 did not have a guard to protect the rotating parts. Employees were exposed to catching their fingers and hand at the unguarded disk. (c) Building 472, Machine Shop: On or about January 14, 2015, a Fosdick Drill Press, Model Radial, Serial Number: 171-051353, Voltage 220, and Amp 8.4 did not have a guard to protect the rotating parts. Employees were exposed to catching their fingers and hand at the unguarded disk. (d) Building 472, Machine Shop: On or about January 14, 2015, a Bridgeport Hardinge Milling Machine, Model Ram, Serial Number 041341, Voltage 230, and Amp 6.2 did not have a guard to protect the rotating parts. Employees were exposed to catching their fingers and hand at the unguarded disk.

29 CFR 1910.242(b): Compressed air used for cleaning. Compressed air was not used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment. Building 379, Machine Shop: On or about January 14, 2015, an employee did not use a compressed air nozzle less than 30 p.s.i. The nozzle was connected to air pressure gauge reading 95 psi. The compressed air was used for blowing or cleaning metal chips inside the HARDINGE Conquest T42 Automation Machine, Part Number N157812M1306-101, Serial Number 7873. Employee was exposed to High-pressure injection of particles or liquids damaging eyes or skin.
