29 CFR 1910.212 (a)(1): One or methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Building 379, Carpenter Shop: On about January 20, 2015, the unused portion of the saw blade was not guarded on the Band Saw, Mach. No. 116-D, Serial #: 60378 and ½ inch side blade, which had approximately 5 inches of the saw blade exposed. Employees were exposed to amputation related injuries.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade were not guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed. (a) Building 379, Wood Working Shop: On about January 20, 2015, the lower right and left side portion of the saw blade on the Delta Radial Arm Saw, Serial Number 55E30619, CAT Number 33-082, 4500 RPM, 14 inch blade, was not guarded. Employees were exposed to amputation related injuries.
