29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    On or about 01/30/2015, in the boiler room where maintenance workers access on regular basis, a stair with eight risers was not equipped with an intermediate rail.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 01/30/2015, in the mold repair shop, a Powermatic Drill press Model No. 1150, Serial No. 815V1041 used on daily basis was used without any guard(s).
