29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite, 41 Allegheny River Avenue, Oakmont, PA: On or about 03/13/15 two employees were unloading boxes of cabinets from a powered industrial truck and moving them on a deck approximately 33 feet above the ground and no fall protection systems were in place to prevent the employees from falling.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): (a) Jobsite, 41 Allegheny River Avenue, Oakmont, PA: On or about 03/13/15 the employer did not ensure that employees who were working at heights and exposed to fall hazards had been trained. Two employees were working at heights up to approximately 33 feet without fall protection and the employer did not ensure the employees had been trained on the nature of falls in the work area and the proper use and operation of guardrail systems, personal fall arrest systems or other methods to be used to prevent falls.
