29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer.   A. T. Davis Contractors, LLC worksite located at 46797 Carronade Dr., Perrysburg, Ohio: On or about March 16, 2015,  the employer failed to ensure frequent and regular inspections of the jobsite were conducted to ensure employees were not exposed to hazards including but not limited to fall hazards.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     A. T. Davis Contractors, LLC worksite located at 46797 Carronade Dr., Perrysburg, Ohio: On or about March 16, 2015, the employer did not ensure employees were protected from fall hazards while installing plywood sheeting on the North side of the 4/12 pitch single story structure. Employees were exposed to a fall distance of 8.50 feet.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. T. Davis Contractors, LLC worksite located at 46797 Carronade Dr., Perrysburg, Ohio: On or about March 16, 2015, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns to secure sheets of plywood to the structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.
