Davis Contractors, LLC, on or about July 12, 2016, at the address of 29181 Fox Creek Dr. (Building #15) Perrysburg, OH: The employer did not ensure employees were protected from head injuries while walking/working below employees operating from aerial lifts above. At least one employee was exposed to potential impact injuries caused from falling material and/or equipment.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) T. Davis Contractors, LLC on or about July 12, 2016, at the address of 29181 Fox Creek Dr. (Building #15) Perrysburg, OH: The employer did not ensure an employee working from an aerial lift was adequately protected from falls. The employee working from the lift was not clipped off to the basket while the platform was elevated. The employee was exposed to falls of approximately 16 feet to the ground below.
