29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  T. Davis Contractors LLC on or about May 31, 2017, at the address of 1470 Pray Blvd., Waterville, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various wood framing operations from elevated surfaces.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  T. Davis Contractors LLC on or about May 31, 2017, at the address of 1470 Pray Blvd., Waterville, OH: The employer did not ensure an employee was protected from falls while performing wood framing activities on top a multi-family residential structure.  During this activity, employees were exposed to falls up to 32 ft. to the ground below.    T. Davis Contractors was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1046799, citation number 1, item number 1b and was affirmed as a final order on June 18, 2017, with respect to a workplace located at 46797 Carronade Dr. Perrysburg, OH.
