Appalachian Wood Products, Inc., 171 Appalachian Drive, Clearfield, PA  16830, maintenance shop area, on or about September 15, 2015 and times prior thereto:  A cart containing an oxygen cylinder and an acetylene cylinder was stored within three feet of a Baldor pedestal grinder (Cat. No. 1400W, Spec. No. S11866).  The Baldor pedestal grinder is used daily to clean parts.

The lockout / tagout procedures for the moulder were deficient in that:  (1) detailed notes, cautions, and/or instructions were not published on the procedure to wait for the cutter to come to a complete stop prior to entering the danger zone.  (2) techniques and/or instructions were not detailed on the procedure for bleeding residual air pressure from the hold down wheels.  (3) the magnitudes for the energy sources were not published on the procedure.
