29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6. Section 3.5.1 of the Compressed Gas Association Pamphlet P1-1965, as adopted by 29 CFR 1910.101(b), states "Do not store cylinders near highly flammable solvents, combustible waste material and similar substances, or near unprotected electrical connections, gas flames or other sources of ignition." A.) Appalachian Wood Products, Inc., 171 Appalachian Drive, Clearfield, PA 16830, maintenance shop area, on or about September 15, 2015 and times prior thereto: A cart containing an oxygen cylinder and an acetylene cylinder was stored within three feet of a Baldor pedestal grinder (Cat. No. 1400W, Spec. No. S11866). The Baldor pedestal grinder is used daily to clean parts.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Appalachian Wood Products, Inc., 171 Appalachian Drive, Clearfield, PA 16830, on or about May 5, 2015 and times prior thereto: Employees performing servicing and maintenance on machines and equipment including, but not limited to, the scmi Topset XL moulder (#796; S/N: AA1/013260). The lockout / tagout procedures for the moulder were deficient in that: (1) detailed notes, cautions, and/or instructions were not published on the procedure to wait for the cutter to come to a complete stop prior to entering the danger zone. (2) techniques and/or instructions were not detailed on the procedure for bleeding residual air pressure from the hold down wheels. (3) the magnitudes for the energy sources were not published on the procedure.

Appalachian Wood Products, Inc., 171 Appalachian Drive, Clearfield, PA 16830, on or about May 5, 2015 and times prior thereto: The rear panels on the scmi Topset XL moulder (#796; S/N: AA1/013260) were equipped with latches that were to be locked / unlocked with a special keyed tool. These specialized latches were not secured in the locked position during machine operations.
