A. Casey Bortles worksite located at 2861 Woods Edge, Perrysburg, Ohio: On or about May 28, 2015, the employer failed to instruct all employees in the recognition and avoidance of hazards commonly seen in the roofing industry. Employees were exposed to eye injuries and improper ladder usage.

Employees potentially exposed to fall hazards were not trained/re-trained in the means and methods of fall protection appropriate to the scope of work.

A. Casey Bortles worksite located at 2861 Woods Edge, Perrysburg, Ohio: On or about May 28, 2015, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns to secure weatherproofing on the two story residential structure. The employees were exposed to potential flying object hazards while operating pneumatic fasteners.  Casey Bortles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.95, which was contained in OSHA inspection number 962398, citation number 1, item number 1 and was affirmed as a final order on April 16, 2014, with respect to a workplace located at Maumee, Ohio.

The employees were exposed to a potential fall distance of approximately 19.00 feet.  Casey Bortles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501, which was contained in OSHA inspection number 962398, citation number 1, item number 2 and was affirmed as a final order on April 16, 2014, with respect to a workplace located at Maumee, Ohio.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  A. Casey Bortles worksite located at 2861 Woods Edge, Perrysburg, Ohio: On or about May 28, 2015, the employer did not ensure the side rails of the extension ladders were extended at least three feet above the landing surface located around the structure. All employees were exposed to fall while using the ladders to access the roof, specifically located on the North and South sides of the two story residential structure.   Casey Bortles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053, which was contained in OSHA inspection number 962398, citation number 1, item number 3 and was affirmed as a final order on April 16, 2014, with respect to a workplace located at Maumee, Ohio.
