29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access.   A) Casey Bortles on or about April 26, 2018 at the site located at 1466 Blackhawk Drive, Waterville, OH: The employer did not ensure a means of access was provided for an employee working from a ladder jack scaffold 17ft above the level below. The exposed employees were climbing the supporting ladders to access and exit the ladder jack platform. More than two feet separated the access ladder rung and the working platform.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  A) Casey Bortles on or about April 26, 2018, at the address of 1466 Blackhawk Drive, Waterville, OH: The employer did not ensure three employees were protected from a fall while working from a ladder jack scaffold platform.  The employees were exposed to a 17ft fall to the level below while installing new roof material to the roof, and or while ascending and descending, to get more supplies from the ground below.
