29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a)  On September 26, 2017 at the development off of Pray Boulevard (The Colony at Waterville Landing), Waterville, OH:  The employer did not ensure an accident prevention program was in place to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).   Casey Bortles, was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1066332, citation number 1, item number 1a and was affirmed as a final order on July 14, 2015, with respect to a workplace located at 2861 Woods Edge, Perrysburg, Ohio.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)      On September 26, 2017,  at the development off of Pray Boulevard (The Colony at Waterville Landing), Waterville, OH: The employer did not  ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, personal fall arrest systems or alternative measures while performing residential roofing operations.     Casey Bortles, was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.503(c), which was contained in OSHA inspection number 1066332, citation number 1, item number 1c and was affirmed as a final order on July 14, 2015, with respect to a workplace located at 2861 Woods Edge, Perrysburg, Ohio.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.   a)  On September 26, 2017, at the development off of Pray Boulevard (The Colony at Waterville Landing), Waterville, OH: The employer did not ensure employees were using eye protection while operating pneumatic nail guns to secure asphalt shingles. The employees were exposed to potential flying object hazards while performing this task.    Casey Bortles, was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 1066332, repeat citation number 2, item number 1 and was affirmed as a final order on July 14, 2015, with respect to a workplace located at 2861 Woods Edge, Perrysburg, Ohio.     Casey Bortles, was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 962398, serious citation number 1, item number 1 and was affirmed as a final order on April 16, 2014, with respect to a workplace located at 7664 Lonetree Court, Maumee, Ohio.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On September 26, 2017, at the development off of Pray Boulevard (The Colony at Waterville Landing), Waterville, OH: The employer did not ensure employees were protected from falls while installing asphalt shingles of a residential type structure.  Employees working in this condition were exposed falls of 8 ft. fall to the ground below.   Casey Bortles, was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1066332, repeat citation number 2, item number 2 and was affirmed as a final order on July 14, 2015, with respect to a workplace located at 2861 Woods Edge, Perrysburg, Ohio.
