29 CFR 1926.20(b)(1): The employer does not initiate and maintain such programs as may be necessary to comply with this part:  A)     Casey Bortles: Employees are performing residential roofing activities without the benefit of an accident prevention program to ensure employees are protected from hazards that can result in eye injuries through the use of eye protection, and from fall hazards of approximately 11 feet to the ground below through proper utilization and use of the fall protection and ladders. This violation was recently observed on or about July 11, 2019, at the location of 1173 Riley Lane, Waterville, OH 43566, where employees were conducting roofing activities.  Casey Bortles was previously cited for a violation of this Occupational Safety and Health Standard (29 CFR 1926.20(b)(1)), which was contained in OSHA Inspection # 1331306, Citation number 1, Item number 1, and was affirmed as a final order on 10/18/2018, with respect to a workplace located at 2842 River Road, Maumee, OH 43537. 29 CFR 1926.503(a)(1): The employer does not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A)     Casey Bortles: The employer does not ensure that employees are adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting construction activities atop the one story residential structure. Employees are exposed to approximately 11 feet fall hazard from the roof of the structure to the ground below. This violation was recently observed on or about July 11, 2019, at the location of 1173 Riley Lane, Waterville, OH 43566, where employees were conducting roofing activities.
