Casey bortles worksite located in Waterville, Ohio:  On or about March 14, 2018, the employer did not provide abatement verification for the following citation items: 1-1, 1-2, 1-3, and 1-4. The citation items became final order on January 11, 2018 with an abatement due date of December 26, 2017.

Casey Bortles worksite located in Waterville, Ohio:  The employer does not initiate and maintain an accident prevention program to aid the employer and his employees in the recognition and avoidance of hazards associated with residential roofing operations (i.e. fall protection and the use of personal protective equipment), as observed on October 26, 2017, at The Colony at Waterville Landing off Pray Blvd.  To abate this hazard, the employer must implement programs to ensure that compliance with construction safety and health standards are complied with.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, are not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a. Casey Bortles worksite located in Waterville, Ohio:  The employer does not ensure employees wear safety glasses when operating pneumatic nail guns to secure shingles during residential roofing operations, as observed on October 26, 2017, at The Colony at Waterville Landing off Pray Blvd.   To abate this hazard, the employer must provide and require employees to use personal protective equipment, such as eye protection, when exposed to flying object hazards.

Casey Bortles worksite located in Waterville, Ohio:  The employer does not ensure employees are protected from fall hazards of 8 feet 10 inches when performing residential roofing operations, as observed on October 26, 2017, at The Colony at Waterville Landing off Pray Blvd.   To abate this hazard, the employer must provide fall protection for employees exposed to fall hazards greater than six feet above a lower level.

Employees are not able to recognize the hazards of falling and are not trained in the procedures to be followed in order to minimize these hazards.  a. Casey Bortles worksite located in Waterville, Ohio:  The employer does not ensure employees performing residential roofing operations and exposed to fall hazards of 8 feet 10 inches, were trained on fall hazards and the procedures to be followed to minimize fall hazards, as observed on October 26, 2017 at The Colony at Waterville Landing off Pray Blvd.    To abate this hazard, the employer must provide training to all employees exposed to fall hazards to ensure that they are able to recognize the hazards of falling and procedures to eliminate or minimize these hazards.
