29 CFR 1910.219(m)(2)(i): Expanded metal, sheet or perforated metal, and wire mesh was not securely fastened to frame.    (a)  Truck Receiving/Loading area on the North end, the center auger for unloading the bin just north of the main leg, next to the rail road, had a broken hinge and latch on the guard and was held together with a bungee cord which allowed it to be readily removable by the operator.  This exposed employees to in-running nip points.      (b)  Truck Receiving/Loading area on the North end, the center auger for unloading the bin just south of the main leg, next to the rail road, had a broken latch on the guard and was held together with a bungee cord which allowed it to be readily removable by the operator. 29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    (a)  In the Scale Loading Area, an electrical disconnect labeled #27 did not identify the purpose of the disconnect.    (b)  In the Scale Loading Area, an electrical disconnect labeled #45 did not identify the purpose of the disconnect.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    (a)  On or about June 15, 2015, in the Scale Loading Area, electrical panels used to power equipment such as #21 west wet bin fan, #22 west wet bin fan, #23 east wet bin fan, LP truck dump, #26 bin sweep, #41 aeration fan east tank NE corner, #13 pit conveyor, and #43 aeration fan west tank NW corner were blocked with a wheat sample machine exposing employees to electrical shock hazards.

29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods:      (a)  On or about April 2, 2015; Bin 5 was entered by employee(s) to sweep out the bin floor did not have all energy sources locked out for equipment such as but not limited to the bottom auger exposing employees to entanglement hazards.    (b)  On or about March 12, 2015; Bin 1A was entered by employee(s) to clean out the bin and did not have all energy sources locked out when performing cleaning activities.  The energy source(s) were just shut off.
