29 CFR  1910.333(b)(2): "Lockout and Tagging." While any employee is exposed to contact with parts of fixed electric equipment or circuits which have been deenergized, the circuits energizing the parts shall be locked out or tagged or both in accordance with the requirements of this paragraph. The requirements shall be followed in the order in which they are presented (i.e., paragraph (b)(2)(i) first, then paragraph (b)(2)(ii), etc.). Note 1: As used in this section, fixed equipment refers to equipment fastened in place or connected by permanent wiring methods. Note 2: Lockout and tagging procedures that comply with paragraphs (c) through (f) of 1910.147 will also be deemed to comply with paragraph (b)(2) of this section provided that: [1] The procedures address the electrical safety hazards covered by this Subpart; and [2] The procedures also incorporate the requirements of paragraphs (b)(2)(iii)(D) and (b)(2)(iv)(B) of this section.  During the week of May 23, 2022, at an establishment located at N1344 County Road W, in Fremont, Wisconsin; written procedures were not developed and utilized by employees when changing fuses exposing employees to electrical shock hazards.

If such hazards are present, or likely to be present, the employer shall:   During the week of May 23, 2022 and thereafter, at an establishment located at N1344 County Road W, in Fremont, Wisconsin; the employer had not sufficiently detailed a hazard assessment and documentation for grain handling employees performing electrical testing and/or troubleshooting activities on faulty fuses exposing employees to electrical shock and/or arc flash hazards.

During the week of May 23, 2022, at an establishment located at N1344 County Road W, in Fremont, Wisconsin; the employer did not ensure the grain handling employee(s) and maintenance employee(s) were utilizing appropriate protective equipment to protect from electrical hazards when changing blown fuses.  Employee(s) used a voltage meter to troubleshoot and were not provided with a fire resistant long sleeve shirt.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  (a)  During May 2022, at an establishment located at N1344 County Road W, in Fremont, Wisconsin;  Bin 41 had a damaged guard on the pulley assembly for the bottom outfeed auger exposing employees to caught in hazards.
