29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  (a)  In the Old System, the permit-required confined space for the elevator leg pit did not have the danger sign(s) in place.  (b)  In the Old System, the two permit-required confined space for the pit located by Bin 4 did not have the danger sign(s) in place.  The Country Visions Cooperative was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.146(c)(2), which was contained in OSHA inspection number 990057, citation number 1, item number 1 and was affirmed as a final order on December 3, 2014, with respect to a workplace located at W2468 Cty E., Chilton, WI 53014.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a)  On or about August 20, 2015, the employer failed to provide the information for employees using respirators when not required under the standard as outlined in Appendix D.
