Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about May 26, 2022, at an establishment located at N1340 County Road W, in Fremont, Wisconsin; in the Agronomy Building; the employer did not obtain written approval from the manufacturer prior to attaching and utilizing a galvanized steel cage for lifting employees. The employer did not have the nameplate changed to address the addition.
