29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). On or about July 20, 2015 at the above addressed job site, an employee performing roof metal sheeting operation working from an aerial lift was not attached to the boom, exposing him to a fall hazard of approximately 26 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about July 20, 2013 at the above addressed job site, an employee was exposed to a 26 foot fall hazard when installing metal sheets on a roof with no means of fall protection.
