29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6. Section 3.4.12, Compressed Gas Association Pamphlet P-1-1965, as adopted by 29 CFR 1910.101(b): Connections to piping, regulators and other appliances were not always kept tight to prevent leakage.   a)    Lyon Shipyard, Concrete Pier: On or about 08/14/2015, an oxygen regulator was connected to the pier manifold that was not tight at the joint between the flash arrestor and hose to prevent leakage.    No abatement certification or documentation required.

29 CFR 1915.55(g)(2): Torches were not inspected at the beginning of each shift for leaking shut-off valves, hose coupling(s) or tip connections.   a)    Lyon Shipyard, Vessel Terrapin Island: On or about 08/13/2015, compressed liquefied petroleum gas (Star-Flame) was supplied from an in-plant tank to a manifold, regulator, hoses, and torch. The torch was not tight to prevent leakage as a result an employee received minor burns resulting from an explosion.   No abatement certification or documentation required.
