29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, pinch points and moving parts: A:) The Tapco Tube Company, M-3, entry section of the tube mill, end welder - On or about 9/21/15, and at times prior thereto, the guide rollers at the end welder were not guarded. Employees were exposed to the condition when performing activities that included but were not limited to taking measurements and conducting welding work from the platform.
