29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as service and/ or maintenance of equipment. (a) In Cut Up Area: A maintenance employee replaced the E-stop on the PRS Prosaw (serial 3552) and specific procedures were not utilized, exposing the employee to electrical shock hazards. The PRS Prosaw machine operates on 240 Volts and 100 lbs. of air pressure. (b) In Saw Area: Affected employees clear jams such as, planks and studs on the conveyor machine and specific procedural steps were not developed for clearing jams, exposing employees to amputation hazards.
