If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.    a) Throughout the establishment:  On or about February 14, 2018, and at times prior, the employer exposed employees to a struck-by hazard, in that forklift drivers did not have a clear forward view while moving loads of pallets throughout the establishment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a) En-trim saws: On or about February 14, 2018, and at times prior, the employer exposed employees to an amputation hazard, in that guards were not placed properly around the saw blades of the en-trim saws.  The guards must be installed so they do not get stuck in an up position.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a) Throughout the establishment: On or about February 14, 2018, and at times prior, the employer exposed employees to an amputation hazard, in that lockout training was not conducted for both authorized and affected employees working on or near machines, such as, but not limited to, en-trim saw, pallet stacker, dismantler, and rotochopper.    Bo's Pallets, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 116896, citation number 01 , item number 02a and 02b and was affirmed as a final order on July 1, 2016, with respect to a workplace located at 2427 Highway 140 NW, Adairsville, Georgia.
