29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.  a. On or about February 9, 2016, located in the fabrication/maintenance building, the employer did not ensure oxygen and acetylene bottles were secured to protect the cylinders from being knocked over, thereby exposing employee to a struck-by hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  a. On or about February 9, 2016, located in the fabrication/maintenance building, the employer did not ensure oxygen bottles were separated with a 20 foot separation or a fire-resistance barrier from acetylene bottles, thereby exposing employees to a fire hazard.

On or about February 9, 2016, located in the fabrication/maintenance building on the left side of the south garage door, the employer did not identify a fire extinguisher so that it would be readily accessible to employees.    b. On or about February 9, 2016, located in the fabrication/maintenance building on the right side of the south garage door, the employer did not identify a fire extinguisher so that it would be readily accessible to employees.   c. On or about February 9, 2016, located in the fabrication/maintenance building where the welding generators were stored, the employer did not identify a fire extinguisher so that it would be readily accessible to employees.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  a. On or about February 9, 2016, located throughout the facility, forklifts were modified with drilled holes in the end of the tines for pulling trailers or below-the-tine hoisting which could affect the safe capacity and operation of the forklift.
