29 CFR 1926.1423(e) For non-assembly/disassembly work, the employer must provide and ensure the use of fall protection equipment for employees who are on a walking/working surface with an unprotected side or edge more than 6 feet above a lower level as follows: 29 CFR 1926.1423(e)(2): While at a work station on any part of the equipment (including the boom, of any type), except when the employee is at or near draw-works (when the equipment is running), in the cab, or on the deck. a. On or about February 12, 2016, located at 6640 Riverside Drive in Dublin, Ohio, an employee was exposed to a fall hazard greater than six feet while performing crane inspections while on the top of a Manitowoc crawler crane, model number 111-B, serial number 1180507.

29 CFR 1926.1428(a)(3): The employer must make the documentation for whichever option is used available at the site while the signal person is employed by the employer. The documentation must specify each type of signaling (e.g. hand signals, radio signals, etc.) for which the signal person meets the requirements of paragraph (c) of this section. a. On or about February 12, 2016, located at 6640 Riverside Drive in Dublin, Ohio, the documentation did not specify the type of signaling performed by each signaling employee when hand/arm signals and radios were used to signal for crane hoisted loads.
