29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 24, 2016, at the above addressed site, an employee was exposed to a fall hazard of approximately 10 feet 4 inches when gaining access to and from a roof while using a 20-foot aluminum extension ladder that was not extended at least 3 feet past the upper landing surface.
