29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Jobsite: The Preserve at Longleaf located at 3231 Conservation Place, Melbourne, FL: Approximately ten (10) employees were exposed to fall hazards in that they were not appropriately trained to avoid unsafe conditions associated with working on a steep roof approximately twenty (20) feet above the ground level with a pitch of approximately 5:12 on or about 12/9/2022.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)   (a) Jobsite: The Preserve at Longleaf located at 3231 Conservation Place, Melbourne, FL: Approximately ten (10) employees  performing residential construction activities such as roof tile installation, on a 20 feet high a 5:12 pitch roof, were not protected from falling, exposing employees to fall hazards of more than twenty (20) feet to the ground level, on or about 12/9/2022.  The G and G Roofing Construction Inc. was previously cited for a violation of a occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1581699, citation number 1, item number 1 and was affirmed as a final order on 09/22/2022, with respect to a workplace located at 4465 Preserve Drive.

(a) Jobsite: 456 Gus Hipp Blvd, Rockledge, FL, on or about 12/15/2022, the employer Run Loss Reports used for recording injuries and illnesses were not an equivalent form for the calendar year 2022, at least 5 recordable injuries were not appropriately recorded. The Run Loss reports were missing the following information: Total number of deaths, Total number of cases with days away from work, Total number of cases with job transfer or restrictions, Total number of recordable cases, Total number of days away from work, Total number of days of job transfer or restriction, Total number of injuries, Total number of poisonings, Total number of skin disorders, Total number of hearing loss, Total number of respiratory conditions, Total number of all other illnesses, Annual average number of employees, Total number of hours worked by all employees at the company, Company address, Brief industry description of the work the company does, and the NIACS code under which the company operates.
