29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)   a) Jobsite: Preserve at Longleaf, at 3241 Conservation Place, Melbourne, FL: employees performing residential construction activities such as roof tile installation, on a 20 feet high a 5:12 pitch roof, were not protected from falling, exposing employees to fall hazards of more than twenty (20) feet to the ground level, on or about 1/12/2023.  b) Jobsite: Preserve at Longleaf, at 3231 Conservation Place, MELBOURNE, FL: employees performing residential construction activities such as roof tile installation, on a 20 feet high a 5:12 pitch roof, were not protected from falling, exposing employees to fall hazards of more than twenty (20) feet to the ground level, on or about 2/20/2023.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Jobsite: 3241 Conservation Place, Melbourne, FL: twenty eight (28) foot extension ladder being used to access the two (2) story roof was not properly installed,  the ladder's side rails extended approximately eighteen (18) inches above the upper landing surface, exposing employees to a fall hazard, on or about 1/12/ 2023.  (b) Jobsite: 3231 Conservation Place, Melbourne, FL: twenty eight (28) foot extension ladder being used to access the two (2) story roof was not properly installed,  the ladder's side rails extended approximately seventeen (17) inches above the upper landing surface, exposing employees to a fall hazard, on or about 2/20/2023.  The G and G Roofing Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1581699, citation number 1, item number 2 and was affirmed as a final order on 09/22/2022, with respect to a workplace located at 4465 Preserve Drive.
